Prior to performing crown lengthening or shortening, we will review your dental and medical history and carefully examine your teeth, gums, bite, and supporting bone. This evaluation allows us to determine why treatment is needed and whether crown lengthening, crown shortening, or another periodontal procedure is the most appropriate option for you. We may also evaluate the area for signs of gum disease, bone loss, tooth mobility, decay, or other conditions that could affect treatment.

Once it has been determined that crown lengthening is appropriate, the procedure will begin with the administration of a local anesthetic and, when needed, sedation. Small incisions are made in the gum tissue to provide access to the treatment area. The excess gum tissue is then carefully removed or repositioned to expose more of the natural tooth. When crown lengthening is performed in preparation for a dental restoration, it may also be necessary to reshape a small amount of the supporting bone so the crown can be properly placed. Once the desired amount of tooth is exposed and the gumline has been contoured, the tissue is repositioned and secured with sutures.

Crown shortening is generally performed to cover an exposed tooth root and improve the appearance of a tooth that looks unusually long because of gum recession. Depending on your needs, gum tissue may be repositioned or grafting material may be placed over the exposed root. The tissue is then carefully positioned and secured with sutures to create a more even gumline and protect the affected tooth.

Crown lengthening or shortening can often be completed in approximately one hour, although the length of the procedure will depend on the number of teeth being treated, the technique used, and whether gum tissue, bone, or both require treatment.

Initial recovery following crown lengthening or shortening typically takes between one and two weeks, although the exact healing time will depend on the type and extent of the procedure. Patients who receive sedation may experience some temporary grogginess, which should generally improve within several hours. A protective periodontal dressing may also be placed over the treatment area to protect the tissue while it begins to heal. It is essential to follow these and any additional post-operative instructions provided by your periodontist to minimize complications and support a comfortable recovery.

• To reduce pain or discomfort, take prescribed medication and/or over-the-counter pain relievers as directed.
• To reduce swelling, apply an ice pack over the area for 10 to 15 minutes at a time.
• Do not spit, rinse forcefully, or use a straw during the first 24 hours.
• Avoid strenuous physical activity and rest as much as possible during the first 24 hours.
• Eat soft foods such as applesauce, yogurt, pudding, eggs, or cooled soup during the initial healing period. Gradually reintroduce firmer foods as directed, and avoid chewing directly on the treated area.
• Continue practicing good oral hygiene, but avoid brushing or flossing the surgical area until your periodontist advises that it is safe. Attend all scheduled follow-up appointments and continue regular periodontal care to protect the health of your teeth and gums.

By following these instructions and keeping the surrounding teeth and gums clean, you can support the healing process and return to your normal routine as comfortably as possible.
